This is the given task of my mentor after my training as a Medical Virtual assistant. I created this form by using Google sheet. This a medical history form basically a questionnaire used by the doctors/clinics to understand your health background before treatment. It is used to avoid drug interactions, spot risk for procedure and make better diagnoses.

It is the Doctors Online Appointment Form it is the digital version of the paper form you'd fill up at the clinic's front desk. It books your slot and gives the doctor key info before you meet. That way your visit starts faster.
